What is CHR&R?
County Health Rankings and Roadmaps (CHR&R) was a program that was run by the University of Wisconsin Population Health Institute. CHR&R focused its attention on why there are differences in health within and across communities. CHR&R has shared an understanding of health, equity, and the power of communities to improve health for all since 2011.
What is CHR&R used for?
CHR&R created resources and tools that support community-led efforts to accurately diagnose core problems, understand and account for historical context and implement evidence-informed solutions. CHR&R provided a snapshot of the health of nearly every country in the nation.
Why are we moving it here?
The County Health Rankings and Roadmaps program will cease its current operations in its present form after December 2026 due to the end of funding from the Robert Wood Johnson Foundation. Many Programs at the Kentucky Department for Public Health and local health departments use this data for tracking health trends, developing community assessments and grant reporting. In order not to lose any of this important data, KDPH has moved all previous year’s CHR&R data to the Kentucky Environmental Health Tracking Portal.
