Access and Functional Needs Assessment

The Access and Functional Needs (AFN) GIS maps were created by the Environmental Public Health Tracking Program of the Kentucky Department for Public Health (KDPH) to enhance current emergency preparedness and response planning. The datasets​ within these maps were created to make location-based community information easy to access by public safety officials across Kentucky. The Emergency Preparedness and Response Branch of KDPH uses various social vulnerability indicators to prepare and respond to disease outbreaks, natural or human-caused disasters.


  • Location Data Map​
    • This map shows location data such as hospitals, health care facilities, and emergency manager contacts. 
  • Age and Population Data Map
    • This map shows age and population data provided by the American Community Survey. 
  • Communication Data Map
    • This map shows data on variables that may impact the way messaging is provided to a community. 
  • Medical and Disability Data Map
    • This map shows data on medical and disability variables that may be important during emergencies. 
  • Other Determinants Map
    • ​This map shows data on other variables, such as poverty and homelessness​, that may be important during emergencies. ​
